Health insurance for Michigan travel nurses: covering contract gaps when the marketplace PPO costs more

What to know first: Michigan offers marketplace PPO coverage statewide, but it generally costs roughly one-third more than the lowest-priced silver option. Losing agency insurance when an assignment finishes gives you 60 days to enroll. Marketplace tax credits typically make those plans more affordable. Healthy nurses earning beyond the credit threshold should also compare medically underwritten nationwide PPO coverage.

You live in Michigan but take hospital assignments across state lines, often lasting several months. The assignment itself isn't always the insurance headache. It's what happens afterward, whether a contract gets cancelled, you take several weeks between placements, or agency coverage disappears alongside your income.

Michigan does have a marketplace PPO option. For 2026, its insurer reports being the state's sole marketplace PPO carrier, while federal plan data confirms availability across all 83 counties. Comparing unsubsidized silver premiums for 40-year-olds within each county, the lowest-priced PPO typically costs about 35% more than the least expensive silver plan overall. County differences range from roughly 9% to 105%, depending on location.

These figures come from federal plan data comparing counties at a single age without subsidies. They illustrate premium differences, not the exact amount you'll owe where you live.

For Michigan travel nurses, I'd start with two questions: are you currently between assignments, and does your annual income qualify you for premium tax credits?

If you're between contracts and coverage just stopped

When your agency insurance ends because your contract finishes, that coverage loss starts a 60-day marketplace enrollment period outside open enrollment. It's losing insurance that matters, not when your assignment begins or ends. You can use this opportunity even with another contract already arranged.

Two state rules are especially useful between assignments. Under Michigan law, short-term coverage cannot exceed 185 days within any 365-day period with the same insurer, and renewals beyond that are prohibited. That limits its usefulness to shorter gaps. Medicaid, meanwhile, considers current monthly earnings. If an extended break reduces your income substantially, check the Healthy Michigan Plan, which covers qualifying adults earning up to 138% of the federal poverty level, before paying an unsubsidized premium.

Don't let your 60-day enrollment opportunity expire. Sam Jaber can compare coverage based on how long you're between assignments and when your next one begins. Call Sam Jaber at 813-999-0101.

If your income earns you a tax credit

If your projected annual earnings qualify for federal premium credits, HealthCare.gov generally offers the more affordable choice because those credits cannot be used outside the marketplace. Seven insurers participated in Michigan's marketplace for 2026, although your county determines which are available.

Michigan offers a somewhat different price comparison than certain other expensive states. In a typical county, moving from the PPO insurer's affiliated HMO to its PPO raises the premium around 21%, approximately $145 monthly for someone age 40, based on their lowest-priced silver options. Subsidies may make that broader network worthwhile for some nurses. Others might choose the least expensive plan and schedule routine appointments during visits home, relying on federal protections that require out-of-network emergency treatment to be covered at in-network cost.

Keep your annual income estimate current regardless of your plan choice. Premium credits are settled when you file taxes, and in 2026 you may have to repay the entire excess amount.

See whether the PPO's added cost makes sense. Sam Jaber can compare premiums for both plans after applying your available tax credit. Call Sam Jaber at 813-999-0101.

If you earn too much for a credit

Federal premium credits disappear above 400% of the poverty level, equal to $63,840 for one person seeking 2027 coverage or $62,600 for 2026. Beyond that threshold, you cover the entire premium. Michigan rates also increased: KFF reports that the benchmark second-lowest-cost silver plan for age 40 climbed approximately 29.5%, from $404 monthly in 2025 to $523 in 2026.

Paying full premiums makes a PPO priced roughly one-third above the lowest-cost silver plan expensive. Meanwhile, a Michigan-focused HMO generally provides mostly emergency coverage when you're working in other states. Your medical situation helps determine whether either choice works.

If you're healthy and over the credit line

Healthy nurses above the income cutoff should also request quotes outside the marketplace for medically underwritten private coverage with nationwide PPO provider networks. These policies aren't HealthCare.gov products, so their premiums aren't tied to what Michigan's marketplace PPO charges.

Compare nationwide PPO coverage against Michigan's unsubsidized premiums. Sam Jaber can provide both prices, and the online form doesn't request medical information. Call Sam Jaber at 813-999-0101.

If you need steady care for a condition

If you're treating an existing medical condition, take extra care comparing plans rather than assuming only one route works. Sam Jaber can review private and marketplace coverage based on your particular needs.

Michigan's marketplace PPO is available statewide, making it the PPO option here that guarantees acceptance. For nurses needing access to specialists who aren't easily reached near home, compare its premium and network with the HMO and private PPO choices before deciding whether the extra cost makes sense.

Michigan nurse questions

What do travel nurses do for health insurance between contracts?

Losing agency insurance when your contract finishes gives you 60 days to enroll through the marketplace. Michigan restricts short-term policies to 185 days per 365-day period with one insurer. If your monthly earnings fall sufficiently, Medicaid could also become an option.

Is there a PPO on the Michigan marketplace?

Yes. Michigan has one marketplace insurer offering PPO coverage for 2026, available throughout all 83 counties. Comparing unsubsidized silver options county by county at age 40, its least expensive PPO typically costs approximately 35% more than the lowest-priced silver plan.

How much did Michigan marketplace prices go up for 2026?

According to KFF, Michigan's benchmark second-lowest-cost silver premium for a 40-year-old increased about 29.5%, from $404 monthly in 2025 to $523 in 2026. Your actual premium varies by county, age and selected plan.

Do I make too much for a tax credit as a travel nurse?

For 2027 coverage selected during open enrollment starting November 1, federal credits disappear above 400% of the poverty level, or $63,840 for one person, compared with $62,600 in 2026. Marketplace premiums then become entirely your responsibility. Healthy nurses should also consider pricing medically underwritten nationwide PPO plans.
