Health insurance for Virginia consultants outside Northern Virginia who need a PPO that travels
The short answer: For 2026, Virginia's marketplace has HMO and EPO medical plans but no PPO outside a few Northern Virginia communities, while full premiums increased about 21.6%. If you qualify for tax credits, marketplace coverage usually costs less. If you're above that range and healthy, it's worth comparing a medically underwritten plan with a nationwide PPO network.

Place me with Sam JaberMaybe you're based in Richmond, Norfolk, Virginia Beach, Charlottesville or Roanoke, but your consulting takes you to Charlotte this month and New Jersey next month. You bill clients through projects or retainers, cover your own insurance premiums, and need coverage that follows your work.
According to CMS's state marketplace plan files, Virginia's Insurance Marketplace has just one insurer offering PPO medical coverage in 2026, limited to a few Northern Virginia areas, including Arlington, Alexandria and parts of Fairfax County. Your home address determines which marketplace plans you can purchase. For the rest of Virginia, that means choosing between regional HMO and EPO networks.
Start by asking yourself: is your household's yearly income within the premium tax credit limits? For a single consultant, federal assistance stops above $63,840 for 2027 coverage selected during open enrollment beginning November 1. The 2026 cutoff is $62,600. Both represent 400% of the poverty guideline, and larger households have higher limits.
If your billings leave you eligible for a credit
If you're launching your practice, dealing with a quiet quarter or still building your client pipeline, your income might qualify for credits. When it does, Virginia's Insurance Marketplace at marketplace.virginia.gov, not HealthCare.gov, generally gives you the lowest coverage cost, even with an HMO or EPO. That's because marketplace coverage is the only place those credits apply.
Predicting income gets tricky when you're paid by project. Regular retainers make forecasting easier, but a major contract signed in November can change everything. Your advance tax credits get reconciled when you file taxes, and earning more than estimated could require returning some or all of that assistance. See the subsidy cliff guide for how abruptly those credits can disappear.
When you're traveling, your Virginia HMO or EPO still covers actual emergencies. Federal requirements make marketplace plans cover out-of-network emergency treatment without advance authorization and at in-network cost. But routine physicals, prescription refills and follow-up appointments generally need to wait until you're back in Virginia.
Your next move: build an income estimate from your pipeline. Sam Jaber can review your retainers and pending proposals, estimate your annual income and help match you with Virginia coverage. Prefer a conversation? Call 813-999-0101.

Once you're outside the tax credit range, you're responsible for the entire premium, which climbed considerably in Virginia. The State Corporation Commission's Bureau of Insurance reported a 21.6% average increase before subsidies in 2026. That happened despite Virginia's reinsurance program, which the Commission says has reported reductions of approximately 15% to 17% annually since starting in 2023.
For consultants earning above that threshold, the result is a higher unsubsidized premium for an HMO or EPO. Routine treatment remains concentrated around home, while care elsewhere in the country is generally limited to emergencies. Many consultants find that difficult to justify financially. Your health determines which alternative makes sense.
If you're healthy and work for clients across state lines
If you're healthy, looking beyond the marketplace may make more sense. Some private, medically underwritten plans provide access to PPO networks spanning the country. Because these aren't Virginia's Insurance Marketplace products, Northern Virginia's geographic restriction doesn't apply. A physician near your Philadelphia client could be in network, just like a provider in Henrico.
What to expect:
- Your medical history determines eligibility. You'll answer health questions during the application, and the insurer can approve your coverage, reject the application or exclude an existing condition. That's what makes this option better suited to consultants who are healthy.
- Medical screening can make the price difference. Because an insurer accepting only people who qualify through underwriting takes on less risk than coverage open to everybody, a healthy consultant may pay less than Virginia's full marketplace premium.
- Look up providers in the cities you travel to. Check the network directory for hospitals, urgent care facilities and primary care doctors across your usual client destinations, instead of searching only Richmond or Norfolk.
- Benefits depend on the individual plan. Coverage outside the marketplace follows its own benefit terms, so understand those details before enrolling. Our guide to medical underwriting breaks down the approval process.
The best way to decide is to put the nationwide PPO's network and premium alongside a Virginia HMO or EPO at full price, using the same household for both quotes.

If you're dealing with a medical condition, don't rule either option out before comparing them. The right choice comes down to your particular circumstances, and Sam Jaber can review those details with you.
For marketplace coverage, you're comparing Virginia's available HMOs and EPOs. Insurers vary depending on your address, and their provider networks and prescription formularies aren't identical. Verify that your specialists and medications are included before weighing premiums, and schedule visits during the weeks you're back home.
Next step: match your coverage to your Virginia providers. Sam Jaber can compare plans available at your address with the specialists you visit and prescriptions you take. Want to discuss it? Call 813-999-0101.
Choose a plan with Sam JaberIf your coverage from a former employer is ending
Consultants often start their own businesses after working for an established firm. At larger employers, federal COBRA lets you temporarily retain group coverage while paying as much as 102% of the total cost. For smaller employers outside federal COBRA, Virginia's section 38.2-3541 of the Code of Virginia permits 12 months of continued group coverage if you had insurance for at least three months before losing it.
Losing either continuation option qualifies you for a special enrollment period through Virginia's Insurance Marketplace. Usually, you can arrange replacement coverage before your existing plan expires and avoid an interruption. If you're healthy with income above the credit limit, include a medically underwritten nationwide PPO in your comparison.
Next step: arrange replacement coverage before your current plan ends. Sam Jaber can compare continuation coverage, marketplace options and nationwide PPO plans while accounting for your termination date. Prefer speaking by phone? Call 813-999-0101.
Plan my switch with Sam JaberIf you live in Northern Virginia instead
Living in an area served by Virginia's marketplace PPO gives you another choice. Because it's marketplace coverage, your health cannot disqualify you, and premium tax credits can apply. But don't assume its network extends everywhere you travel. Check its reach beyond Northern Virginia, then compare its price with an underwritten nationwide PPO if you're healthy and earn above the credit threshold.
Next step: put the three choices side by side. Sam Jaber can review a Northern Virginia marketplace PPO, an HMO or EPO, and a medically underwritten nationwide plan together. Prefer a conversation? Call 813-999-0101.

Open the two-minute checkQuestions Virginia consultants ask
Can I buy the Northern Virginia marketplace PPO if I live in Richmond?
No. Your residence determines marketplace eligibility, not the places you work. Virginia's only marketplace PPO insurer in 2026 serves a limited number of Northern Virginia communities. Even visiting clients in Arlington every week won't make those plans available at an address outside the service area.
Is private health insurance better than the marketplace for a consultant?
Your income and medical situation are the deciding factors. Qualifying for tax credits generally makes marketplace insurance more affordable. If you're healthy and above the eligibility limit, an underwritten nationwide PPO could offer better value. Having a continuing medical condition doesn't automatically eliminate private coverage, since eligibility depends on your circumstances.
How much did Virginia marketplace premiums go up for 2026?
The State Corporation Commission's Bureau of Insurance reported an average increase of approximately 21.6% before subsidies. Premiums rose that much despite Virginia having operated a state reinsurance program since 2023.
Do I make too much for Obamacare in Virginia?
For a single person, the federal premium tax credit ends above $63,840 in income for 2027 coverage, with open enrollment starting November 1. That equals 400% of the poverty guideline. The corresponding 2026 limit is $62,600, while bigger households have higher thresholds. Virginia is also introducing state assistance for 2027 targeting households between 138% and 250% of the poverty level.
